About The Role

This is an exciting new opportunity to join our support team in Cornwall as a Support Worker in our Complex Needs Community Outreach service based in Truro.

Based in Truro you will work across mid-Cornwall visiting people in their own homes who require support. You will be lone working with people who have complex needs including substance misuse, mental ill-heath any other challenging issues and those who are struggling to maintain their accommodation or are facing homelessness.  Proving a holistic support service assisting with budgeting, benefits, health and wellbeing, housing, maintaining tenancies, sign-posting and emotional support. You will need to take a creative approach to support, adapting your style of communication and support planning with each individual to draw up a person centred support plan and help customers establish and work towards their goals and aspirations.

You will need to be able to effectively risk assess, have a good understanding of safeguarding and be able to develop strong links with other services and providers that support children and families.

You will be expected to work flexibly to ensure that the needs of the clients are met and this may include late shifts and some weekend working.

This role is being offered on a part time basis, working 22.2 hours per week, over 3 days to include weekend working.
