About The Role

We’re pleased to advertise an opportunity for a Support Worker to join our Supported Housing team at the New Horizons Scheme. This role is ideal for colleagues who are passionate about delivering person‑centred support and helping customers to build independence, confidence and stability within their communities.

New Horizons provide emergency supported accommodation for people and families at risk, often with complex support needs. The team supports people needing to flee domestic abuse but who wouldn’t be able to access communal living refuge accommodation.

As a Support Worker, you’ll work directly with customers who have a range of complex needs, supporting them to achieve agreed outcomes in a flexible, responsive and strengths‑based way. No two days are the same, and you’ll need to be comfortable working outside a traditional 9–5 environment.

Key responsibilities include:

  • Delivering high‑quality, person‑centred support tailored to individual needs
  • Building positive and professional relationships with customers, colleagues and partner agencies
  • Supporting customers to identify goals and develop personal support plans
  • Promoting independence, inclusion and participation to reduce social isolation
  • Completing risk assessments and managing risk in line with policies and procedures
  • Maintaining accurate records and contributing to service performance targets
  • Working safely, including lone working, while maintaining clear professional boundaries

Why should I apply for this role?

  • This is a great opportunity to develop your career within LiveWest, building on your existing knowledge and experience
  • You’ll join a supportive and collaborative team that works together to achieve positive outcomes for customers
  • The role offers variety and autonomy, allowing you to make a real difference in people’s lives
  • You’ll continue to strengthen your skills in supported housing, risk management and person‑centred support
  • It’s an opportunity to take on a new challenge while staying within an organisation whose values you already know and share

This is a full time role working 37 hours per week – working pattern is Monday - Friday, with one weekend worked every six weeks.

This is a field based role covering our services across mid - west Cornwall.

We are committed to safeguarding and promoting the welfare of vulnerable groups. Appointment to this role is subject to a satisfactory enhanced DBS check.
